Support Coordination
Support Coordination is the most common service under this category. Your Support Coordinator helps you understand your NDIS plan and use it effectively. They connect you with providers, set up service agreements, troubleshoot issues, and prepare you for plan reviews. Think of them as your guide through the NDIS system—answering questions as they arise and keeping you one step ahead.
Specialist Support Coordination
Some participants face more complex situations that require a higher level of expertise. Specialist Support Coordination is delivered by practitioners with clinical or specialist backgrounds and is used when a participant’s situation involves significant risk, complexity, or the need for clinical oversight. Not everyone needs this level of support, but for those who do, it can make a significant difference.
School-to-Adult Life Transition
Leaving school is one of the biggest transitions in a young person’s life. For young people with disability, it can be especially difficult without early planning. Our team helps families and young participants plan well ahead of the final school year.
We help map out goals, identify the right supports, and ensure everything is in place before the school structure disappears. Ideally, transition planning begins at least two years before the final school year.
NDIS Plan Reviews and Reassessments
A plan review is a major event in any participant’s NDIS journey. We help participants prepare by documenting progress, identifying unmet needs, and building strong evidence for required supports. We can also assist in organising plan variations to ensure your supports align with your goals. A well-prepared plan review can make a real difference.

As you age or experience changes in your health, your support needs may change. We help participants and families review what is in place, identify gaps, and adjust the NDIS plan accordingly.
We also assist participants approaching 65 to understand their options as they transition from the NDIS to the aged care system.

Moving into your own home or shared living is an exciting milestone—but it requires careful planning. We help you explore housing options, set up daily living supports, and make sure your NDIS plan reflects your new living situation. We also assist with:
