Assist-Life Stage, Transition

NDIS Support Category 0106 | Capacity Building Supports

Life doesn’t stand still. School ends. People move out. Health changes. Plans get reviewed. These are big moments, and they can feel overwhelming for participants and families alike. At Serene Life, we help you navigate those moments with confidence with someone in your corner who knows the NDIS inside out and genuinely cares about what comes next for you.

What Is Assist-Life Stage, Transition Support?

Assist-Life Stage, Transition (NDIS Support Category 0106) helps you manage major life changes. It sits under Capacity Building Supports in your NDIS plan.

The most common service under this category is Support Coordination. A trained coordinator helps you understand your NDIS plan, connect with the right services, and manage your support day to day.

It also covers specialist support for people going through specific life changes such as leaving school, moving out of home, entering aged care, or adjusting to a new diagnosis.

Transitions are the moments when participants are most at risk of losing ground. A young person finishing school without a solid plan, an adult whose informal carer can no longer help, or someone whose health has changed and whose current supports no longer fit. This support exists to ensure these moments do not become crises.

Our team includes experienced Support Workers, Registered Nurses (RNs), and Enrolled Nurses (ENs) who bring care and understanding to every transition. Families and healthcare providers trust us to deliver consistent, personalised support—including the comfort of working with the same familiar faces throughout your journey.

What Does Assist-Life Stage, Transition Support Include?

Support under this category varies depending on the life stage or transition you are going through. Here is what we offer:

Support Coordination

Support Coordination is the most common service under this category. Your Support Coordinator helps you understand your NDIS plan and use it effectively. They connect you with providers, set up service agreements, troubleshoot issues, and prepare you for plan reviews. Think of them as your guide through the NDIS system—answering questions as they arise and keeping you one step ahead.

Specialist Support Coordination

Some participants face more complex situations that require a higher level of expertise. Specialist Support Coordination is delivered by practitioners with clinical or specialist backgrounds and is used when a participant’s situation involves significant risk, complexity, or the need for clinical oversight. Not everyone needs this level of support, but for those who do, it can make a significant difference.

School-to-Adult Life Transition

Leaving school is one of the biggest transitions in a young person’s life. For young people with disability, it can be especially difficult without early planning. Our team helps families and young participants plan well ahead of the final school year.

We help map out goals, identify the right supports, and ensure everything is in place before the school structure disappears. Ideally, transition planning begins at least two years before the final school year.

NDIS Plan Reviews and Reassessments

A plan review is a major event in any participant’s NDIS journey. We help participants prepare by documenting progress, identifying unmet needs, and building strong evidence for required supports. We can also assist in organising plan variations to ensure your supports align with your goals. A well-prepared plan review can make a real difference.

Ageing and Health Changes

As you age or experience changes in your health, your support needs may change. We help participants and families review what is in place, identify gaps, and adjust the NDIS plan accordingly.

We also assist participants approaching 65 to understand their options as they transition from the NDIS to the aged care system.

Entering or Leaving Hospital or Residential Care

A hospital admission or a move into residential care can significantly disrupt NDIS supports. Our team coordinates transitions in and out of these settings, ensuring supports are ready when the participant returns home or moves to a new arrangement.

Moving Out of Home

Moving into your own home or shared living is an exciting milestone—but it requires careful planning. We help you explore housing options, set up daily living supports, and make sure your NDIS plan reflects your new living situation. We also assist with:

  • Understanding tenancy agreements and housing obligations
  • Building practical everyday skills such as cooking, budgeting, and household management
  • Creating daily routines that support independence
  • Staying involved during the settling-in period to make sure everything works smoothly

Skills Development and Community Connection

Big changes shouldn’t mean losing touch with the people and things you love. Alongside practical support, we help you stay connected and continue growing. This includes:

  • Learning public transport and improving communication skills
  • Mentorship, peer support, and goal-setting
  • Connecting with local groups, events, and communities
  • Decision-making, daily planning, and budgeting support
  • Transport access across Sydney

Who Is Assist-Life Stage, Transition Support For?

Assist-Life Stage, Transition support is for NDIS participants experiencing major changes and needing guidance. It is also for families who want to ensure their loved one is prepared for the next stage of life.

Young People Finishing School

The final years of school are the ideal time to start planning for adulthood. Young people with disability—and their families—need time to explore options, set goals, and put the right supports in place. We help families start early so the transition is planned, not rushed.

Participants New to the NDIS

Getting your first NDIS plan is exciting, but it can feel overwhelming. Support Coordination helps new participants understand their plan, choose providers, and use their funding effectively. A good start sets the tone for long-term success.

Participants Whose Plans Are Not Working

Sometimes a plan doesn’t reflect a participant’s real needs. Supports may be missing, providers may not be the right fit, or funding may not match daily life. We help identify what isn’t working and support you to make necessary changes.

Participants Approaching 65

Turning 65 often means transitioning from the NDIS to the aged care system. This is one of the most complex transitions. We help participants understand what changes, what stays the same, and how to ensure appropriate supports continue.

Families Managing Support for a Loved One

Families often carry a heavy load when coordinating support. Support Coordination significantly reduces that burden. We manage logistics, provider relationships, and NDIS paperwork—allowing families to focus on their relationships.

Participants With Complex or High Needs

Participants with complex needs often have multiple providers and detailed support arrangements. Specialist Support Coordination provides dedicated, skilled oversight to manage complexity and advocate effectively.

Frequently Asked Questions

Is NDIS employment support the same as Disability Employment Services?

No. DES is funded by the Australian Government and is free to access. NDIS employment support (0102) is funded through your Capacity Building budget and focuses on  disability-specific barriers that DES does not always cover. Many participants use both, and we can help you set up both if they are right for your goals.

Do I need to already have a job to use this support?

No. This support covers both accessing and maintaining employment. You can use it at any stage – whether you have never worked, are looking for your first job, returning after time away, or trying to stay in a role you currently have.

Can this support help me if I want to work part-time?

Yes. Many participants work part-time, casually, or in voluntary roles. We focus on what is realistic and meaningful for you – not a fixed idea of what employment should look like.

What if my employer is not willing to make adjustments?

Under the Disability Discrimination Act 1992, employers must make reasonable adjustments unless doing so would cause genuine hardship. We can help you understand your rights, prepare for that conversation, and escalate to the Australian Human Rights Commission if needed. We do not leave participants to manage this alone.

What skills can I develop to prepare for work?

Time management, workplace communication, and using assistive tools are key areas. These can also be supported through Development-Life Skills (0117), which works well alongside 0102. Your plan manager can confirm what you have available.

What types of jobs can Serene Life help me find?

We help participants across a wide range of roles and sectors in Sydney. We start with your interests and what you are already good at, then help you find roles that match your skills, disability needs, and goals.
